Introduction

To prequalify with SHAPE, please enter your details into the fields below and click ‘Submit Details’. A DocuSign form will then be emailed to you for completion.

After your initial prequalification, we will ask you to re-verify that your information remains current every 12 months. You will be prompted to do this through an automated email reminder. Previous responses will be pre-populated into the DocuSign form to make the re-verification process as quick and seamless as possible for you.

Any Entity, Insurance, Banking detail changes need to be directed to accountspayable@shape.com.au.
